From what your saying, it sounds like you're not pressing "apply" after choosing an effect. The apply button is what puts the effect on your photo. If you, for example, add a sepia effect, then directly afterwards select "wrinkle remover" without pressing the apply button, a box will appear on your screen asking if you want to discard the sepia effect, since you haven't actually applied it yet.
As for the upgrading, some effects are unavailable to you unless if you get a premium account. You have to pay for a premium account, though. I have a picnik premium, and the price is actually not that bad for all of the cool effects I can use. Still, if you're just using Picnik to do simple editing, you really don't need a premium account.
